Goal: The Mobile C.A.R.E. Foundation’s mission is to provide complimentary and comprehensive asthma care and education to children and families in Chicago’s underserved communities via mobile medical units called "Asthma Vans."

Impact: The Mobile C.A.R.E. Foundation’s Asthma Vans provide children and families in Chicago’s underserved communities with complimentary and comprehensive asthma care and education resulting in reduced school absenteeism, decreased ER visits and lower hospitalization rates.

Goal: The goal of MDFT is to reduce adolescent drug abuse and increase self-efficacy in the teen population.

Impact: Systematic reviews comparing the effective of adolescent drug use interventions across studies found that MDFT reduces substance use, delinquency, behavior problems, and symptoms of anxiety and depression. The program has also been found to improve educational performance.

Goal: NCDC participants are committed to reducing diesel emissions and finding innovative ways to protect human health and the environment. To fully address the challenges of reducing diesel emissions the NCDC is using a multi-pronged approach:

- Commitment to the successful implementation of the 2007 Highway Engine Rule and the Clean Air Nonroad Diesel Rule.
- Developing new emissions standards for locomotive and marine diesel engines.
- Promoting the reduction of emissions for existing diesel engines through cost-effective and innovative strategies, including use of cleaner fuels, retrofitting and repairing existing fleets, idling reduction among others.

Goal: The goal of the New Jersey Department of Health's voluntary fluoride mouth rinse program, "Save Our Smiles" is to improve the oral health status of high need/risk children residing in areas where the water is not optimally fluoridated.

Goal: The Northern Michigan Diabetes Initiative is a regional collaboration dedicated to prevention, early detection, and management of diabetes. The Healthy Family Backpack Program connects with youth and their parents to educate participants on proper nutrition and promote healthy lifestyles to reduce childhood obesity.

Impact: The Northern Michigan Diabetes Initiative has distributed nutritional education materials to over 300 families. Of ninety-two families that set a healthy goal at the start of the program, forty-five continued to maintain that goal at the two-month mark.

Goal: The goal of NICHE is to achieve systematic nursing change that will benefit hospitalized older patients. The vision of NICHE is for all patients 65 and over to be given sensitive and exemplary care. The mission of NICHE is to import principles and tools to stimulate a change in the culture of healthcare facilities to achieve patient-centered care for older adults.

Goal: NAP SACC aims to improve policies, practices, and environments in childcare through better nutrition, increased exercise, and staff-child interactions.

Impact: Intervention centers are more likely to make significant changes in nutrition policies, environments, and practices. The intervention has been replicated in other states to help improve nutrition and physical activity policies and practices.

Goal: The main objective of Pool Cool is to increase awareness, motivation, and sun protection practices among children ages 5-10 who take swimming lessons.

Impact: The Pool Cool program had significant positive effects on children's use of sunscreen and overall sun-protection habits at swimming pools.
